Need feedback for a survey on urban sanitation

Hello,

As part of my PhD I am developing a rapid participatory risk assessment tool for urban sanitation systems. Data on the reality of the sanitation situation in many cities in low income countries is often limited.  Municipalities do not have the resources to do detailed surveys of households and the local level sanitation systems.  The tool asks communities to evaluate their situation and health risk against a number of indicators. 

There is limited scientific data which quantifies the relative risk to human health posed by different poor sanitation systems and practises and therefore it is difficult to weigh the different factors into an overall risk score.

I am interested to understand your perceptions of these risks. I would be grateful if you could fill in the questionnaire. It should not take morethan 5 minutes. It contains a bit of information about yourself and 3 questions relating to different elements of risk; sanitation systems, exacerbating factors and vulnerability.
