Borewell failure & chances of recurrence in Barisal, Bangladesh

I am facing a problem with a deep tube well (961 feet) installed in Bakergonj upozilla of Barisal district, Bangladesh in 2006. But after installation it worked just for 2 weeks and after that there were no water from it. Details regarding the borewell are as follows:

 - 4 inch boring was done and 37.5 mm dia pipe is used to collect the water from acquifer.
 - the rotatry cutting method is used which is done manually to bore the hole.
 - the depth of borehole was 1091 feet.
 - only a 20 feet casing pipe was used during boring.a 20 feet pvc filter is used to collect the water from the acquifer.
 - the tubewell was installed in 20.08.06.it lost workability after 2 weeks.

We showed the tubewell to DPHE engineers as well as to our own engineers.no one could tell what was the reason behind this. We are planning to dig another tubewell there but I want to understand the reason behind the failure & its chances for recurrence.
More or less every tubewell in the location possess a depth around 1000 feet and the closest one to our tube well is in the vicinity of around 600 feet.
