Borewell casing pipe stuck at 60m in Nepal : Need advice on causes and remedies

The borewell was drilled by percussion method up to 116 m and extended after few days up to 125 m.  The casing pipe could be lowered only upto 60 m, after which it is blocked. Further tries could not improve on the situation. The driller suggests a reduction in the diameter of the casing pipe, slotting pipe and pump size.

The location of bore is Chitwan district of Nepal. The Narayani river flows along the this district goes to meet Gandak Barraage. It is the inner tarai belt of nepal. It the border of Bihar state of India.

1. What are possible causes of not being able to lower the pipe more than 60m?
2. How to know the the borewell is vertically straight, plumb?
3. Is internal damage a reason for the current situation?
4. What remedies can be implemented for the lowering of the casing pipe?
5. The pipe size of ERW is 12" and 10" and current suggestion is to reduce the pipe size to 10" and 8" respectively. if pipe size is reduced  
      to 10" and 8" will it result in further issues in the future?
